Kayako and Toshio's curse continues by haunting The Grudge 2's only survivor Jake (Matthew Knight) who is now confined to a psychiatric hospital after his family's alleged suicide. As Jake's case worker Dr. Sullivan (Shawnee Smith) investigates his Chicago home the curse continues to claim more victims. The third instalment in the series of horror films based on the Japanese original. A young Japanese woman, Naoko (Emi Ikehata), seems to be the only one who can stop the curse of Kayako (Aiko Horiuchi), so she travels to the Chicago apartment of Jake (Matthew Knight), the sole survivor of the last film. There she also meets a family who are being tormented by Kayako and Toshio (Shimba Tsuchiya). Somehow, the mysterious stranger must find a way to banish the seemingly unstoppable spirits forever.
